Well it is based off a book about the actual person. Though it's probably exaggerated in order to sell more.
They do touch a bit on the video censorship stuff, but censorship in Japan is from some old law from the 1900s IIRC. It doesn't exactly say genitals are bad, just how the heads of government choose to interpret it or something.
Porn in Japan isn't banned because they don't have the hangups and guilt about it that most of us, having grown up in a culture that at some point in the past has been heavily influenced by Christianity, have.
Japan's declining birthrate is nothing special. Portugal, Spain, France, England, Wales, etc. all have declining birth rates which is normal for developed countries where most young people find having a kid to be not financially feasible. The only reason we think it's a big deal is because Japan likes to toot its own horn and make it sound like they are super special.
The most common reasons that Japan doesn't have enough kids is because most people do not wish to have kids as they can't afford them, or they have careers that will end up derailed if they have kids.
It's definitely not because they aren't having enough sex or anything like that. Japan has a lot of sex, just look at all the prostitution business' around train stations. There wouldn't be a ton of them if there wasn't a demand.
So yeah, a lot of couples are having sex, just not with their husbands / wives. The usual occurrence is, once they get married and have a kid, roles shift and the wife becomes a mother who now dotes over the little one and gives little to no time for sex. She does all the housework while daddy is off pretending to work hard in front of his bosses to provide for the family. Though of course the husband surrenders his pay to the wife as she controls finances, and he gets a weekly allowance from her.
This allowance tends to cover the cost of a prostitute.
Most Japanese do not think it's cheating if you pay for it. As long as there are no emotions, it's not cheating.
If you do cheat and find a mistress, it's ok as long as your spouse doesn't find out about it. Don't ask don't tell, because your spouse likely has her own lover too, but make sure it doesn't follow you home because you can get sued for like a couple million yen if adultery comes to light.
But that's what happens when you marry someone because society dictates that you need to be married by 30. People who may not exactly be compatible with each other getting married to conform to society's expectations and just shikatanai'ng whatever issues.
So yes, you would be surprised at how much cheating happens in Japan, and how it is viewed as normal.
Japan is super repressed, and most people think they're super polite nice guys because of what they show you, but they're just like the rest of us, humans.
Edit: Oh also prostitution is illegal in Japan but since it's defined as something like vaginal intercourse with an unknown person for cash or something, it's basically just loophole heaven. So anal sex is fine, but also if you met a prostitute at a brothel, then paid for a massage wink wink, and got to know her through the massage so you guys are now acquainted, I guess vaginal sex is ok.

Japanese TV shows are usually very subpar in my experience but this looked a bit fun in the trailer! Also helpful that the people I saw in the trailer are actual actors as far as I know, not idols getting acting gigs (also very normal in Japan sadly).
Yeah, generally their tv shows are trash mostly because no one wants to rock the boat by doing something fresh since it's targeted at house wives who have the purchasing power for whatever it is their sponsors want to sell.

yes most of their laughing shows are fake as fuck to, some very cool exceptions but all the laughing tapes and the stunner prentending to be dumb girls and the hunk cool looking boys there, damn, world of fakenes oozes from it sadly. But when they have foreign stars visit some of these gameshows, i do have a good laugh.

Yeah, that's one of the loopholes since they can argue that on the way to the hotel they got to know each other and sex was agreed to between both parties and the massage parlor had nothing to do with it. Nevermind that it's a 300 dollar massage or something.
There will be an occasional raid but that's mostly for show as most of these places are owned / have ties to the Yakuza who in turn are in bed with politicians.
Then there's the host clubs which has young men drinking tons of alcohol a night (and forcibly puking it up) as they make cash based on how much overpriced booze the women who visit them purchase. Imagine paying 200 dollars to get in, then hundreds of dollars for a bottle of cheap spirits or thousands for the actual good ones.
Hosts basically guilt trip their clients into spending more, and most of the time these are office ladies or students. How do these girls manage to spend thousands of dollars a week? Rich parents? Business owner? Maybe, but most likely they resort to prostitution or one of the other water trades likes a hostess or something.
They should make a show about that as well.
And yeah, alcohol consumption is one of the ways most people use to get through working 80 hours of unpaid overtime a week, even having to come in on their day off. Though the reason they spend so long in the office is mostly because they don't do anything for most of the day and pretend to be busy and wait for the boss to come in before they start working as they have this weird notion of it being rude to go home before the boss.
Then there's how most traditional Japanese companies frown on efficiency. If you get your work done early, the rest of the day where you're on standby waiting for tasks means you're wasting company money because they see it as they're paying you to not do anything.
And since the most affordable drinks are happoshu (low malt beer that is not really beer), the hangovers are brutal. Dunno how so many of them can manage getting up on time the next day.
Well, taking all these topics on this page while adding a little madness to the formula I highly recommend Takashi Miike's Visitor Q. Here's a clip about the movie but it basically tells the entire premise and show plenty of the stronger scenes, ie. it's a spoiler. Watching up until 2:55 is safe, then it starts dissecting the movie. For squeamish people or people who are not fond of japanese weirdness mixed with irony mixed with violence mixed with abuse and so on should watch neither the clip nor the movie. To me Visitor Q/Bizita Q was a revelactation.

Yep luckily this show is really good, seen 3 eps so far and it's really binge worthy.
