Ichi the killer is in the same vain as fight club though. It's a bit more graphic but not really a headfuck once you realise the 'twist'
Besides you said anything by miike... I wouldn't call DOA or izo very confusing.

There arent many movies I have been unable to stomach but there was something about Audition. It was on tv not long ago and I had to turn it off. I watched one foot being sawn off but my stomach was churning by the time the second foot was being severed. I think it was just noise of the bone and razor wire. I found it highly disturbing.

some people consider sin city to be pretty fucked up. the friend i went to see it in the theatre with just couldn't hold still everytime some got his balles ripped of or something, for me it didn't do much because i knew what was coming because i read the comics, but if you don't expect that i can understand it's pretty hard.
My brother saw it a couple of days ago and he was just very confused because of the chronology. you actually have to think while you watch this movie so you can put the pieces together yourself and now in what order the events occured.
The movie brazil is pretty disturbing too, you watch this movie for 2 hours, and when it ends all i could think was: wtf did i just see?

Natural Bron Killers wasn't bad eithe. (the whole get raped by daddy scene but put in form of a sitcom, the riot in jail, ...)
I always wondered what this movie would have looked like if it had been directed by Tarantino himself instead of Oliver Stone. All I know is that Tarantino was not to happy with the end result.

I don't know if this movie really fits into the catargory, but has anyone ever seen the movie called ' Route 66' or something like that, it has the guy who plays cyclops in the xmen movies.
I just stumbled apon it one day, though it was a good movie bit different.
Its not to old either

myeah true there's more gore in the second one and some ideas are beyond everything you can think of (like raping a dead kid in a bathtub full of shit, cut some hole in some girls belly and then fuck the freshly made hole)
but after seeing the first one your resistance against extreem violence will go up so 2nd one didn't have such a great impact on me

for me "The Hitcher" was quite disturbing, not sure if for you guys. I don't need to see eyes being sliced and ants going out from open wound like in Andalusian Dog (one of toeffy's link) to know that the movie is disturbing, actually with age that things don't work for me anymore.
