With the chance of coming off as a psycho serial killer, i've always found the games with slightly over the top dismemberment to be rather enjoyable at times. Games like Soldier of Fortune where you could just tear off an arm or leg with a well placed shot and the enemy would still try and shoot you in the face.
Looking for games where weapons have some "OOMPH" and preferably where it's not just a "critical kill" where when the enemy die they sometimes lose a limb, but where they keep fighting until properly taken care of.
Also the games where there is an "impale gun", like the Stake Gun of Painkiller, the Crossbow of Half Life or the harpoon thingie the Predator had in Alien vs Predator, where you could just nail a guy to the wall or the ground.
And lastly i am also looking for games with "intelligent hitboxes" i honestly can't remember if i played a game where there wasn't only 2 categories of hitboxes, the critical one (the head) and the rest, doesn't matter if i shoot a guy 2 times in a vital organ of the torso or i shoot him twice in the foot, it does the same damage. I'd like to be able to either shoot him in the arm/hand to disarm him, shoot him in the leg to cripple him so he walks/runs with a limp, or just go for the extra damage of the torso, not just the classic headshot to do more damage.
I haven't played many FPS since Half Life 2, so i've missed out on alot of the newer games, i heard Fear 3 had some "OOMPH" to the weapons, but i am not very fond of the scary games, played a little of the first one and i shat my pants when some shitty little girl was coming at me after some nasty horror build up

dead island was one where the baddies kept fighting after losing body parts (directors cut mod recommended)
also, die by the sword was hilarious when baddies sometimes tried to come at you without arms, hopping on one leg
that wasn't very realistic "gore" though
fallout3 had a bunch of gore too but only on death
But yeah - Dead island. You can break zomies arms with blunt weaopons and then cut them of with knifes etc :O

Soldier of Fortune is pretty old but did well on the gore part, Vivisector is a fairly average game but also has a highly detailed gore system.
Manhunt and especially Manhunt 2 are quite gory too, unsure how the SaW game is but it's probably more puzzle oriented with some gore when you fail.
Horror games also use this in various forms, Doom 3 for example.
(Or that particular section in Quake 4, not that it's a horror game at all.)
Older titles like Fallout 1 and 2 and stuff (Or way back to Wolf 3D) were also quite violent, as were stuff like the Crusader games or several Build engine titles (Blood in particular and it's - quite poor - sequel though Shogo which is pretty brutal too is much better.)
There's a lot of violent titles but I can't think of many that does gore very well other than the usual action or shooter title, even stuff like Soldier of Fortune which allows you to take apart enemies is a bit goofy in comparison, horror titles is probably the best bet.
(Or mixed genres like Manhunt.)

Goldeneye on the N64 is the only game I've played that comes close to your description
I was expecting a lot more games to copy that idea but they never did

Kenshi has this, and even better, even you as the player can take damage in for example the right arm and it will just hang lifeless until you get it fixed, or if you get hurt in the right foot or leg you will become very slow and limp. I wish more games were like that, if an enemy can be damaged in a certain way, the player should too..
