It's quite simple really, I got the achievement, here's how to do it:
| Spoiler: | Run straight through the door and open the balcony door to your right. Vito will say he'll need some kind of rope, and Leo says that he should get one from the laundry basket. Exit the balcony, and run straight again, on your left, near the wardrobe is the laundry basket. Press E and the cutscene appears. Henry never knows that you helped Leo. |

You need to own at least one DLC, mod the content file in it to include everything from the others by a lot of copy/paste work, that should make it load the content and add the stuff while still being "Made Man" or whatever, impossible otherwise as it needs authentication.
(Either use Steam version with achievements or Skidrow proper with DLC and no Steam requirements as such, savegames are compatible though as it's profile based you will need to copy them over to the other profile, some random number representing Steam ID or such I'd guess, C:\Users\UserName\Appdata\Local\2K Games\Mafia II\ Savegame\ - The profile file at top includes overall progress and unlocks like pinups and posters, 01 save is the current game and 100 and the others are chapter specific)

just finished the game , shitty ending , theres no free roam in the city .
good game , but happy i didnt spend money on it.
