I'm talking gameplay wise, not due to technical problems etc
Here's my pick:
Ground Control II on HARD difficulty w/ 1.0.0.7 patch
Far Cry on Veteran
COD on Veteran
Escape from Monkey Island
URU: Ages Beyond Myst

well ever made I don't know since I don't play a lot of games. But the most diffucult I played is probably Adventures of Lolo 3 on NES.
COD on veteran?? well maybe it's because I am a COD addict but I didn't found it that hard.
Reality is an illusion caused by the lack of alcohol.

Republic The Revolution was really tough on its hardest setting. Far Cry on difficult was really tough.
