It does sound like a bit of brush off to be honest, albeit in a rather extreme manner. However, if she is serious perhaps you could just be a friend for now and help her out a bit? If she is telling the truth, perhaps you are the only person she has told? If so try and help her, she could of course just be attention seeking. Only you can tell if she is genuine, up to you what you do. But worrying about your own safety, when someone is (possibly) in dire straits is a tad selfish. But, up to you in the end my friend. It really depends how close you are to her and how much support you can give.
After all, would you abandon your best mate if he told you this? I doubt it. Good luck.

I helped an old friend girl that was paranoid - she thought every car passing by her house was a friend of hers wanting her attention and stuff.
Used my influence over her to get her to see a therapist. Later the therapist told me that she has a condition, and while it's treated that I should stop any contact with her, so that she doesn't include me into one of her fantasies.
Later I've seen her, she's better but I lost my interest meanwhile.

The old "never stick your dick in crazy" proverb fits perfectly here. I'd steer very clear of her.
My brother's last girlfriend was a nutcase as well (but not on a "hearing voices" scale) - whenever we got anything planned and we went to pick her up, she'd not open the door or answer the phone - and this for HOURS at a time. Over the course of a few months, she did this about 15 times - when going to the movies, going to a BBQ, going to an amusement park, etc. etc. After the 4th BBQ she was invited to where she wouldn't answer her phone causing us all to be really late, I was fed up and said I'd never invite her again. I urged my brother to get rid of her because she was clearly not right in the head. Nice girl but extremely unreliable and she did the dumbest of things all the time, wasting money while she's older than me and has never had a job in her life! At least my brother (eventually) got the point and broke it off.

TBH i would leave her too.
I mean it's nice that she is honest about her state (if it's really true).
But reading about the voices in the head just sounds very crazy to me. It wouldn't look so bad to me, if she described it a bit better/not so crazy sounding.

Huh, what makes you upset? It was just a way of saying: No, you should probably not get into a relationship with a girl who is in a mental hospital because she hears voices etc.
About the responsibility part. If she's a really good friend (like years of friendship) then I'd say you can feel responsible for her, since friends support each other. But since you only know her so much, I guess you dont feel responsible or do you?
Also not abandoning her as a friend, there is a chance you might fall in love anyway.

That's because one is far more emotionally draining than the other. Even cancer, as bad as it is, is no contest for many mental illnesses. Cancer is something people mostly know and understand and can deal with but mental illnesses can manifest themselves in millions of ways and are much harder to get a grip on and are often a life-long problem.
Especially dealing with someone you care about who has sudden mood swings where he or she wants to end their life can be extremely taxing on the mind and I speak out of experience here. It's a constant worry, day in day out, that anything might happen at any moment to the point where every phone call you get, makes you nervous and worried that it might be bad news.
Basically, you don't want to get involved in such a situation if you don't have to - and you often can't do anything anyway or actually make it worse (especially when she feels she's a burden to you and feels guilty).

Something seems strange about this. Are you absolutely sure she has done this or are you taking it at face value?
Maybe I've seen too many movies/shows but I was wondering whether this was some twisted test of loyalty of whether you'll stick through thick and thin...

If you really want to you can try to be friends. I wouldn't go into anything too serious with her. I doubt anyone who hears voices in her head telling her to kill herself can be a reliable relationship partner.

I'm gonna be honest. I think you made up your mind before you even wrote the first post. You came here to see if people would judge you and say you're a terrible person for your decision. You're not, life is hard and if you can avoid problems it's usually a good idea. Just don't give this girl any hopes of a relationship (maybe you don't have to say you don't want her because of her issues, say you met someone else, idk).
