With one hand!
Serious: I'd say 90% of the time Fullscreen and the other times it can be windowed/borderless window, but those moments are never serious gaming... more half assed/casual gaming.

Gaming is a serious business for me, not only I always play fullscreen but I also turn off all the lights/close the windows for better immersion *posts the grumpy curtains dude*

Fullscreen always. Not sure why anyone would go for Windowed mode.

Borderless. Love the seamless alt-tabbing, it's supported in almost every game now (and for those where it's not, there are third-party utilities), and I'm yet to see it have any noticeable impact on fps.

Always fullscreen, sennhesiers on, curtains and all doors to the man cave closed. Nothing needs monitoring on my pc.
Only time i even take the headphones off is when i'm playing an indie rogue like with music, or a racing/fighting game, something where in game sounds are irrelevant to the game experience.

I'm yet to see it NOT HAVE noticeable impact on the framerate, which is why I don't use it. So yeah, fullscreen all day, every day.
