So I have a new system for two weeks now.
i5 2500k
8gb ddr3-1600
120gb ssd
gtx560ti
i'm using two monitors, unfortunately the gtx has only 1x hdmi, 1x dvi, 1 x vga.
i connected my acer s273hl via hdmi and my old ass belinea artistline via vga (i also got a vga2dvi and hdmi2dvi adapter so i tried varying a bit.
the problem is my primary monitor (acer) blacks out every now and then for say 1-3 seconds then goes back to normal. i checked connections, updated gfx/monitor drivers. my most recent genius idea was to record with fraps until it happens and i noticed that there is no blackout on the video. so i conclude the problem is monitor related.
can i be sure that it is a hardware malfunction/defect or is there something else i should check?

Try disconnecting the secondary monitor and only use the primary.

Sounds like an HDMI handshake issue...
You might check if there is a firmware update for your monitor (unsure about how Acers update firmware, but my Samsung has a usb port on it to allow for it). And you might make sure that you are running the most current WHQL listed drivers for your GPU. Werelds will most definitely be of more assistance than I can.
It could also be a, "Display driver nvlddmkm has stopped responding and has recovered," issue (A.K.A. TDR error). Although, you'll usually get some sort of message on screen. If you are running Vista or Win7, you can check this through the Event Viewer in the Administrative Tools menu inside of the control panel. Then, just do a search (find) for NVLDDMKM and see if there are any that coincide with your blackouts.
I know this probably isn't much consolation, but I hope that it helps.

is the monitor old?
if it's a monitor hardware issue outside drivers and cabling, it could be the backlights or power supply capacitors dying
i've replaced both myself with good results, but i wouldn't recommend it if you're not comfortable with a soldering iron and delicate hardware
but better be damn sure it's not driver/cable related first
fix it up to a laptop or something through a vga cable and see if it still happens
if it does, it's hardware

first of what is the length of ur HDMI cable? if its over 2-3 meters it can cause issues.. also if its a cheap lead you can change it to find out if its that, its been known to happen, where u get black outs and green dots on screens when its a cable issue..
worth a go first before digging deeper.

When exactly does this happen?

This just started happening to me (about 4 days ago) and only when playing specific games (Alan Wake, E.Y.E. Divine Cybermancy). Fixed it by underclocking. Event viewer shows 2 dozen of these errors, I think I recall having one or two of these errors with this card, but never over two dozen over a period of 4 days. Changing the drivers didn't help - went from 285.62 to 295.73.
These games are basically unplayable now with normal clocks because eventually I get a blue screen...

You might try uninstalling the drivers from safe mode, then run driver sweeper/cleaner (I can't remember exact names at present) from within said safe mode to fully uninstall them. Then do a clean install of whatever drivers you want (I always do this when updating to newer drivers, and I use the clean install option to ensure that it takes care of any pesky registry entries that might not have been caught by the driver sweeper/cleaner).
When I used to get tons of these errors I would do that and usually go back to a more stable driver release. I know that with a lot of the newer cards, they are undervolted and can cause this issue and a couple of other, weirder issues. You might check and see if there is a newer video bios for your gpu or might look into over-volting your card. With the latter option, take heed that you don't go nuts with the voltages as you can definitely fry your card very quickly. I've overvolted all three of my 580's, but they are watercooled, so heat isn't an issue.
Hope this info helps!

Yeah, I did use driver sweeper, tried 4 different drivers too, didn't help. I did -finish- Alan Wake by underclocking, though the problem persists after about ~13 minutes of furmark (on stock clocks, the PC either freezes or displays the error after only 2 minutes). This looks like a more deep-rooted problem unfortunately.
I'll try to reformat, eventually, if that fails, well, this card has served me well. I was looking to replace it soon anyway, though not sure if I'll go with nVidia next time, what with the flood of these errors on their side I've discovered by googling (which has provided no clear fix; or even an idea of a one).

It almost sounds like an undervolting issue now. But since the problem is new, you could just have a card with a death-wish. I know that the stock voltage on my 580's wasn't enough, so I got the EVGA Precision software (afaik, the evga software only works on evga cards) and upped the voltage slightly, after reading through their forums about what voltages worked for certain rigs in certain situations. I can't for the life of me remember what voltages I'm running (since I had to do this a year ago). But, it could be something like that. Just do what I do, buy newer and more! Who needs two kidneys anyway? Amirite?!?

seems it was the hdmi port of the gtx.
tried a different hdmi cable, tried the second hdmi port on the monitor. didn't help.
then i switched to good old vga cable and it didn't happen again.
since i don't use the monitors internal speakers anyway i'm not missing out on anything, right?
or are there any disadvantages?
