That facial expression (And the toilet setting.) makes it seem like that "meme" or parody of that Kuso Misu Technique yaoi (gay) manga (cartoon.) though I'm not certain but it's pretty much spot on for that facial expression.

I have never understood this social trend as well, most people are ok with watching a guy falling of a bridge, watching a car accident, where the car becomes completely destroyed, etc etc, but the moment, someone sheds 2 drops of blood, then it becomes immediately an explicit and horrifying video.
Even in those fail compilations on youtube, there are many clips which resulted in serious injuries or death, and people laugh about it, but if you see a guy cutting a superficial layer of flesh and shedding a minimal amount of blood, then all of a sudden it is classified as to shocking to be seen.
