Hmm I did some of those pranks. Replacing someone's background with a picture of an AutoCAD window that of course wouldn't close. The man restarted his PC a couple of times and then went home.
One I duplicated but never used in the wild:
Replacing some bytes in the command.com so if the accessed A: drive was empty, DOS would display: Your girlfriend is pregnant
(A) Abort, (R) Retry, (I) ignore
Back to Win95, besides the PowerToys there were also the KernelToys. One of those could change the keys on the keyboard. Like changing y to n. Are you sure you want to delete this folder? (y/n)...
