Well I'm baffled with this problem and have spent the last week trying different idea's.
Got a capture card installed (Pinnacle DV500) on a computer with Premiere 6.5 (cant go any higher due to no SSE2 instructions in the processor).
It's been fine for the last 3 years not one problem, but now when loading into Premiere it freezes after "ZigZag" has loaded.
I soon discovered it was the "DV500 Overlay" driver causing this due to the fact if I disable it Premiere loads fine, just with no capture card - which is a problem.
I've tried the following with no luck:
1. I've Reinstalled Premiere 6.5
2. I've Reinstalled DV500 Drivers
3. Reset Premiere 6.5 preferences file
4. Updated the graphic card drivers
5. looked at the IRQ settings for the card
6. Changed the capture card to another PCI slot
The only thing I can now think of is to reinstall windows (which is a last resort) or update the BIOS of the motherboard which is a no go due to managing to destroy a mobo in the past that way.
Having similar problems, I reinstalled premiere all together and it solved it....
If you haven't tried that, do that.

Yeah tried that, Premiere loads fine once the driver s disabled in device manager... I've also tried updating the drivers but they're the latest version. Hmmm...
