OK, I think I'd qualify also as web designer. Did not do any total conversion of phpbb til now though, but I think that shouldn't be a biggie either. Anything special you'd like to see for the page?
Downloading PHPBB3 and it's GDK right now and will take a look at it.

TBH, I'm lately a fan of less is more and try to be very minimalistic regarding the design, so whatever I might come up with, might actually be considered quite shitty
So I'm currently playing around and just testing stuff and didn't touch any of the phpbb styles right now, but as far as I can see, it shouldn't be all too complicated...
Here an idea I had for the header:

Don't mind the shitty code or the missing content ... or the missing functionality
(works so far in chrome, opera, and mobile shizzles, no ie)
Once you are logged in, it should display your username instead of account and on click reveal your menu to log off or access your profile and whatelse is possible.
A search field'd be always present at the top right with the option to go to advanced.
Once logged in, you'll also get an icon for the PM and a quick link to the newest posts in the header. Clicking on the logo should give you the option to switch to the forum/nfo's or reviews.
TBH I'd love to style the forum itself a little like wp 8 but again, I guess not everybody is a friend of that
And I'm not sure if that "shine" on the logo should even stay...

That would be the aim as well. More work design wise in that aspect than it is code-wise. So by all means

Its linux and apache with mysql. Tried, tested and free.
IDE's are a matter of taste and what you are accustomed to. PHP has some great ones too, PHPed to mention one. As far as visual development does, I find that they get in the way of getting a web site done and generate all hell HTML/CSS wise. It is best done by hand.

Yes. A private repository. Sounds nice, although for only five users. The thing is I much rather use a repository under my complete control, including the server. Call it picky, I call it being in control.
