NOt enogh power on front port?Does hard requare Y cable?If yes you must use back ports of motherboard.Front ones are connectod to just one port from motherboard and if hard requare two for extra power front ones will not work because they are practically one..

Cause #1 of failing / malfunctioning front ports: bad grounding. Got front audio as well close to the USB ports? Stick in a pair of headphones, then stick something into the USB ports - bad grounding will present itself as feedback (high pitch, sounds sorta like Morse code).

I had a problem with connecting a mic as well, probably due to some grounding like you say otherwise I'm really clueless. I did the cable management thing in my case few weeks ago and while doing that I reconnected the header cables quite a lot. I shall test now and report back.

Well it doesn't have to be the wiring, it could be that the PCB with the ports in your case is broken or somehow the metal from the ports is touching a metal part of your case.
I'm sure Fractal will replace it if that's the case. I know NZXT do at least, a number of the PCBs for those ports in Phantoms were broken and NZXT replaced them all free of charge, world wide
